Medical and Liability Release

I give my permission for my child(ren) to participate in all activities during the 2025-2026 school year with Transfiguration Lutheran Church. I will not hold the employees or volunteers of TLC responsible for sickness or injuries that may occur while my child(ren) is participating in, or being transported to/from, these activities. If medical care is necessary, I grant permission for a licensed medical professional to provide necessary treatment. I expect to be contacted at the earliest opportunity. I hereby release TLC and its event organizers, officers, directors, agents, employees, volunteers, and affiliated organizations from and against any and all liability arising out of, or in any way connected with my child’s participation in TLC events.

Media Consent

By participating in TLC ministries, I hereby give permission to Transfiguration Lutheran Church to publish photos, videos, and/or audio recordings of me and my child(ren) and family through any and all forms of media. Our image and/or voice may be used at a later date for newsletters, church-related marketing, or other publications—including our website and social media accounts.

I ONLY give permission for photographs of my child to be displayed in the building at TLC.

Family Covenant

All are welcome at TLC. In order to create a safe and healthy environment for everyone, we—as a community of faith—agree to:

1. Respect the rights of all participants and cause no harm to others emotionally, physically, verbally, spiritually, or in any other way.

2. Respect and follow the requests of all adults, leaders, and Peer Ministers, as long as those requests don’t hurt or put us at risk.

3. Respect the property of all facilities and the personal property of others.

4. Refrain from using or possessing drugs, alcohol, tobacco products, firearms, or weapons of any kind while attending any and all TLC-affiliated events.

Families serve as the foundation for faith formation. At Transfiguration, we strive to partner with families in fulfilling the promises made at baptism—promises made by parents, sponsors, and the congregation alike.
